ll film inizia dove il capitolo precedente si era concluso, con Alice e gli altri prigionieri dell'Arcadia attaccati dal plotone Umbrella, capitanato da Jill Valentine. Alice spara al pilota di uno degli aerei, che fa precipitare l'aereo sull'Arcadia: l'impatto è piuttosto forte e Alice cade in acqua, priva di sensi.Alice pare risvegliarsi in una tranquilla casetta della periferia americana, dove vive sposata con Carlos, dal quale ha avuto una figlia, Becky. Ma gli zombie bussano alla porta o, per meglio dire, l’abbattono. Alice riesce a fuggire ma la attende un secondo risveglio, questa volta all’interno di un quartier generale della Umbrella. Evadere è tanto pericoloso quanto necessario.
